The selected population health intervention chosen is the Community Health Outreach Worker Program in the county of Onslow in North Carolina. The intervention has been designed to reach the populations who are at higher risk of getting diseases and reduce the disparities and risks involved. The Community Health Outreach Worker Program is a partnership between the community health department and East Carolina University (Connaughton-Espino & Reese, 2021). The program mainly aims at providing outreach services and education to the Onslow County residents who are at risk of developing chronic diseases like hypertension, diabetes, and obesity. The program also provides the residents with services that will help in the cessation of using tobacco and provide support to pregnant families and women with younger children. The Community Health Outreach Worker Program addresses the social health determinants by providing services that will improve access to healthcare services, resources on healthy living, and education on healthy practices.

Pregnant women and families with young children are also offered counseling and mentorship programs to help them overcome any kind of challenges they have. The levels of prevention and accessibility of the program will be high since the healthcare team will work together to make sure the program works and all those involved get the care they need.
Gap and Recommendations
One gap in the outreach program is that it does not address the needs that the LGBTQ community may have. This is a -problem because most LGBTQ members have mental health problems due to acceptance issues that they have (Connaughton-Espino & Reese, 2021). The recommendation, in this case, would be enhancing the health outcomes to meet the different needs that the LGBTQ community has. This can be done through the program partners with LGBTQ organizations at a local level to train the social workers on topics like transgender health, HIV/AIDS prevention, and mental health.
