Instructions

Before beginning this assessment, make sure that you have completed the three Sentinel U case study activities included in the assessment. These will form the basis of your assessment.

For this assessment you will select one of the mental health Sentinel U case study activities and create a concept map that outlines the pathophysiology, pharmacology, and physical assessment of the mental health diagnosis from your chosen case study.

The key components of a mental health diagnosis concept map include the pathophysiology, pharmacology, and physical assessment of the diagnosis. The concept map should outline the key concepts related to the diagnosis, including risk factors, signs and symptoms, diagnostic procedures, medications, complications, nursing diagnoses, and nursing interventions. The map should be organized using a nursing concept map template, such as spider maps, hierarchical maps, or flow charts, to organize the information. The concept map should be accurate, complete, well-organized, and use appropriate nursing concepts and terminology.

To complete your concept map, you may wish to follow these steps:

  1. Select one of the mental health Sentinel U scenarios:

Mental Health Diagnosis Concept Map

Mental Health Diagnosis – MAJOR DEPRESSION DISORDER (F33.1)

Pathophysiology

Major depressive disorder is a multifaceted condition whose pathophysiology can be explained by various mechanisms, including neurotransmitter imbalances (particularly serotonin, norepinephrine, and dopamine), neuroendocrine factors (low cortisol levels), neuroinflammation, and genetic and environmental factors. Severe early stress can cause neuroendocrine and behavioral responses, especially in susceptible individuals.

Risk Factors

MDD risk factors include family history (first-degree relative’s history of depression), biological factors (neurotransmitter and hormonal imbalance), personality traits such as low self-esteem, chronic stress, traumatic events and experiences, lack of social support, medical conditions such as chronic illnesses, and lifestyle factors such as alcohol and substance use and poor nutrition (Marx et al., 2023).

Pharmacology

Pharmacologic management of Major Depressive Disorder involves the use of antidepressant medications. The medication classes used for MDD include the following:

-Selective Serotonin Reuptake Inhibitors such as fluoxetine (First-line)

-Serotonin-Norepinephrine Reuptake Inhibitors (SNRIs) (such as Duloxetine)

– Tricyclic antidepressants (TCA) such as Amitriptyline and Imipramine

-Monoamine Oxidase Inhibitors (MAOIs) such as phenelzine

-Atypical antidepressants such as Bupropion (Qaseem et al.,2023)

Diagnostic Procedures

Diagnosis is based on the DSM-5 criteria (Kim & Park, 2021)

-At least five depressive symptoms (depressed mood, anhedonia, weight loss/change, insomnia or hypersomnia, fatigue, poor concentration, feelings of worthlessness or excessive guilt, and suicide/death ideations)

– At least 4 of the following in addition to the depressive symptoms: 1. Distress or impairment in social, occupational, and other areas of functioning. 2. Symptoms not attributable to substance or medication use. 3. Symptom not explained by other mental health issues. 4. No history of mania or hypomania.

-Episodes must also cause clinically significant distress and have occurred within two weeks.

Physical Assessment (Signs and Symptoms)

Physical symptoms of major depressive disorders are varied but include the following: psychomotor agitation or retardation, tearfulness, inattention to personal appearance, constricted affect, depressed mood, irritability, changes in sleep patterns, reduced motivation, impaired insight, poor judgment, poor memory, unexplained aches and pain, suicidal thoughts or ideations, anhedonia, and poor concentration (Marx et al., 2023). The signs and symptoms may differ from individual to individual.

Complications

It is a leading cause of disease burden and disability worldwide, with impairments in functional and interpersonal relationships. Other complications include comorbid anxiety disorders and substance use disorders, aggravation of medical comorbidities, and development of self-destructive behavior (Bains & Abdijadid, 2023)

Nursing Diagnosis
  1. Hopelessness related to social isolation and chronic stress (husband announced divorce a few months ago)
  2. Ineffective coping related to overwhelming life stressors (recent divorce) and inadequate resources, as evidenced by verbalized inability to cope and change in routine activities
  3. Imbalanced nutrition less than body requirements related to loss of appetite and fatigue as evidenced by verbalized tiredness and 10lbs recent weight loss
  4. Social isolation related to loss of social support from marital relationship as evidenced by loss of interest in activities, including walking around the block.
Nursing Interventions
  1. Establish a therapeutic relationship and encourage participation in support to provide emotional support and address isolation and hopelessness (Woods et al., 2021)
  2. Teach the patients stress management techniques, including deep breathing and mindfulness meditation, to improve coping.
  3. Promote positive thinking using cognitive behavioral therapy, helping identify and challenge, and address/change negative thoughts.
  4. Provide resources and referrals, including community resources such as counselling and financial and legal support.
  5. Encourage a small, frequent and balanced diet meals, provide nutritional education to the patient, and monitor nutritional intake, tracking of the dietary intake and weight (Khiroya et al., 2023)
  6. Provide emotional support by listening to the patient empathetically to help the patient feel valued and supported.
  7. Administer medications prescribed, educate patient on medications to enhance adherence and effectiveness to improve patient outcomes.
