Assessment briefing
Complete partially completed care plans for patients who are undergoing imaging and living with dementia, alongside the relevant case study of the patient. You are expected to complete as a radiographer point of view or relate to medical imagining to
1) complete the care plan as appropriate and Critical evaluation(justification) of the completed care plan for the selected patient.
2) develop a departmental resource to align with the completed action plan. (You have creative freedom to create any departmental resource that you see appropriate.)
In the body of text, please include an evaluation of your departmental resource, ensuring you have added the resource within the assignment. All elements of the assignment will count towards the word count
Required sections:
The assignment should include:
· An introduction
· A completed care plan
· A critical evaluation/justify of the care plan, where the decisions made for the patient are discussed and critiqued.
· Creation of a departmental resource that aligns with your care plan
· A critical discussion/justification of the departmental resource that has been created
· The departmental resource needs to be included
· A conclusion
Total word counting 3000 words:
Complete care plan and evaluation/justification- (2500 words)
- Create department resource: -(500 words) separately paragraphs. Describe the resource (for example: create a simple poster, or picture of a toy, or anything is realistic and relevant to clinical practice). Also explain how to use and why you choose what you have designed as resource and align with the care plan you complete.
Learning Outcomes needs to be demonstrate:
- Critically evaluate the care plan (complete by you) for a patient living with dementia
- Create a resource to support the imaging of those living with dementia
Advice from module team
Plan your work carefully and ensure that both the care plan and departmental resource are informed by current evidence, policy, and best practice guidance relating to dementia care and imaging. The strongest submissions will demonstrate a patient-centred approach, showing a clear understanding of how individual needs influence imaging care. Use the case study to justify your decisions and ensure that links between the care plan and departmental resource are explicit throughout the assignment.
Tips for success:
· Focus on the individual needs of the patient and how these influence imaging care and decision-making.
· Justify all actions within the care plan using appropriate evidence and best practice guidance.
· Critically evaluate your decisions rather than simply describing what was done.
· Consider communication, consent, dignity, safety, person-centred care, and the imaging environment when developing the care plan.
· Ensure the departmental resource has a clear purpose and target audience.
· Explain how the resource supports the needs identified within the care plan.
· Critically discuss the strengths, limitations, and potential impact of the resource on patient care and departmental practice.
· Use current literature, policy documents, and professional guidance to support your arguments throughout.
Students who have done similar assessment before recommending that you…
· Remember to include the departmental resource within the assignment submission.
· Think creatively when designing the resource, but ensure it remains realistic and relevant to clinical practice.
· Select a resource that directly addresses a need identified within the care plan.
· Start the care plan first, as this often helps identify the most appropriate resource to create.
· Keep referring back to the marking criteria throughout the assignment to ensure that all required elements are addressed.
Care Plan 1: MRI Lumbar spine
Goal: To complete the MRI examination safely while reducing anxiety and maintaining image quality.
Assessment: patient appearance
· Patient has dementia with episodes of confusion and wandering.
· Difficulty remaining still for prolonged periods. (why this is problem, and how can we do to overcome this and why we choose this way to overcome this)
· Becomes suspicious of unfamiliar healthcare environments.
· Moderate hearing impairment affects communication.
Action plan for patient: (just for this procedure)
Needs includes justification-what is the problem, why this is a problem, what is the interventional to overcome the problem, and what overcome you expect to overcome the problem.
Complete Sample care plan: you can create your own or improve this one to use.

Case Study 1 – MRI Lumbar Spine: Lewy Body Dementia
Patient Profile:
- Name: Mr. Arthur Bennett
- Age: 79
- Diagnosis: Lewy Body Dementia
- Living situation: Lives with daughter who provides daily support.
- Referral: MRI lumbar spine for investigation of worsening lower back pain and suspected spinal stenosis.
Background:
Arthur experiences fluctuating cognition and occasional visual hallucinations. He can become confused when routines change and is particularly anxious around medical equipment. His daughter reports that he responds well to clear eye contact, reassurance, and simple one-step instructions.
Presentation on Arrival:
Arthur appears uncertain about why he is attending the hospital. He repeatedly asks whether the scanner is “safe” and expresses concern about being left alone. He frequently changes position in the waiting area and appears distracted by surrounding activity.
Requirement to follow throughout:
Introduction and conclusion to the assignment: Clear, concise introduction outlining aims and context; conclusion synthesizes key findings and implications with critical insight.
Completion of the care plan with reference to the associated case study : Care plan completed comprehensively with excellent integration of case study details and strong clinical rationale
Critical evaluation of the completed care plan for the selected patient: Exceptional critical evaluation demonstrating deep insight, evidence-based justification, and clear understanding of patient needs.
Creation of departmental resource: Resource is innovative, clear, highly relevant, and professionally presented; shows excellent understanding of departmental needs.
Critical discussion and justification of the departmental resource: Sophisticated critical discussion of the resource, demonstrating excellent justification of its design and content through reference to evidence and best practice. Insightfully evaluates how the resource addresses identified patient and departmental needs, acknowledging strengths, limitations, and potential impact on patient experience and imaging outcomes.
Structure and organization: Demonstrates an excellent standard of academic writing with a clear, coherent, and logically structured argument throughout. Sections and paragraphs are well organized and effectively signposted, enabling ideas to develop fluently and critically. Harvard referencing is accurate, consistent, and fully compliant with University of Worcester guidance.
Quality and use of reference sources (intext citations and reference list): minimum 25 references. Goes well beyond what is taught in reading/researching and use of academic sources and references to inform independent insight and understanding
