As a baccalaureate-prepared nurse, your participation and leadership in intra- and interdisciplinary teams will be vital to the health outcomes for your patients and organization. One way to approach designing an improvement project, and implementing change, is to become knowledgeable about the challenges that the organization is facing. To do this, you will interview a healthcare professional. This can be a peer, a nursing colleague, administrator, or someone who could provide you with sufficient knowledge and information on a particular problem or concern within the organization.

It would be an excellent choice to complete the Plan-Do-Study-Act (PDSA) Cycle activity prior to developing the report. The activity consists of four questions that create the opportunity to check your understanding of best practices related to each stage of the PDSA cycle, which is one of a number of change models you can read about. Interview Summary

I interviewed S.T., a fellow nurse and a unit manager at the outpatient department of a 40-bed healthcare institution. The institution offers inpatient and outpatient primary care services and various specialized departments, including psychiatric, oncology, pediatrics, and an integrated maternal and child health clinic. The interviewee oversees nursing services in the outpatient department, ensuring smooth operations, patient care coordination, and nursing staff management.

The other roles include shift scheduling, offering patient care, clinical oversight, monitoring infection control and patient safety measures, and maintaining compliance with protocols and hospital policies/regulatory standards at the unit. S.T. also plays a crucial role in interprofessional collaboration by working with physicians and other specialists to coordinate multidisciplinary care.

The relevant issues identified at the organization from the interview were role confusion and overlap among the interprofessional team members, issues with power dynamics and hierarchies, and cultural/professional differences within the team. S.T. noted that the organization has been taking action to address these issues through interdisciplinary workshops and training.

However, the solution has not been effective since participation in these trainings has not been consistent, especially for nurses with busy schedules, which is why the issues are still experienced. The organization maintains a culture of collaboration and values interprofessional teamwork. It encourages teamwork and ensures every professional’s input is recognized and respected.

I employed several interview strategies to ensure sufficient information was gathered from the interview. First, I prepared adequately before the interview, defining the interview objectives and preparing a guide to ensure all essential questions were captured. Secondly, I began by establishing rapport with the interviewee. I also asked open-ended questions and clarified my questions when necessary.  

Issue Identification

One of the most alarming issues raised from the interview that would require an interdisciplinary approach is the cultural and professional differences in the team, contributing to further barriers to effective collaboration and decision-making. Cultural differences in interprofessional teams manifest in variations in communication styles, beliefs about healthcare, and how illness and treatment are perceived across cultures (Hopf et al., 2021).

Professional differences manifest in differing training backgrounds, terminologies and priorities among professionals, hierarchical dynamics, and varying ethical perspectives. The primary reason why this issue requires an evidence-based approach to address it is because when left unaddressed, the differences further contribute to miscommunication, conflicts, and role confusion, all of which compromise interprofessional team effectiveness. Additionally, there is a need to foster a culture of cohesion for effective interprofessional teamwork, aligning with the organizational culture of teamwork and collaboration.     

Change Theories That Could Lead to an Interdisciplinary Solution

Various potential change theories could lead to an evidence-based interdisciplinary solution to address cultural and professional differences in the team. These theories include Lewin’s change management model and Kotter’s 8-step change model. Barrow and Annamaraju (2022) note that change theories help align change initiatives in healthcare, ensuring they are structured, reducing change resistance, and promoting success in change effectiveness.

 Kurt Lewin’s change management model is a structured approach to organizational change that occurs in three stages: unfreezing, changing (transition), and refreezing (Adelman-Mullally et al., 2024). The unfreezing stage entails creating awareness of the problem or undoing the current equilibrium. The changing/moving stage entails seeking alternatives, demonstrating the benefits of change, and decreasing the forces that negatively affect change. The refreezing stage entails integrating and stabilizing a new equilibrium into the system so that it becomes a habit. This model would effectively address entrenched cultural and professional differences by systematically guiding teams through the change process.

Kotter’s change model provides an 8-step comprehensive framework for implementing organizational change (Zomorodi et al., 2024). The steps are establishing a sense of urgency, forming a coalition, creating a vision for change, communicating the vision, removing obstacles, generating short-term wins, consolidating gains and producing more change, and anchoring new approaches in the culture. This model will help create a solution to address the issue of cultural and professional differences in the institution strategically, since following it will guide the change team from identifying the importance of addressing the issues, addressing barriers that would arise in the change process, and finally incorporating change into routine practice.

The change theories above have been supported by literature and found effective in change projects in healthcare organizations. Moreover, the sources used to support the change theories by Adelman-Mullaly et al. (2024) and Zomorodi et al. (2024) are peer-reviewed sources published in credible journals under change-related topics, hence providing credible and relevant evidence to support the change theories in this case.

Leadership Strategies That Could Lead to an Interdisciplinary Solution

Servant leaders, on the other hand, prioritize team well-being, actively listen to concerns, and support professional growth. Hence, by valuing each team member’s contributions to the team, these leadership styles could help break down hierarchical barriers, strengthen teamwork, and cultivate an environment where cultural and professional differences enhance, rather than hinder, collaboration.

Collaboration Approaches for Interdisciplinary Teams

Effective interdisciplinary collaboration is crucial for addressing cultural and professional differences within an interdisciplinary team. One of the best-practice approaches to address this issue is interprofessional team training programs. According to Warren and Warren (2023), implementing training interventions that focus on team-building, communication skills, and understanding diverse professional roles enhances interdisciplinary collaboration and helps overcome team differences. Such programs prepare healthcare professionals to work cohesively, effectively addressing cultural and professional differences.

Furthermore, promoting open dialogue to develop respect for diversity within the team could help address professional and cultural differences within an organization (Bagga et al., 2023). Open dialogue entails communication and active listening. Creating an environment that encourages open communication allows team members to express opinions, concerns, and suggestions freely. Active listening and valuing each member’s input are crucial for effective problem-solving and conflict resolution, leading to improved collaboration and mutual respect. These approaches are supported by credible literature evidence from peer-reviewed current sources.
